The program is designed to meet the needs of facilities and individuals who have recently had a stay in hospital or with qualified nursing staff and are now returning home. The goal of this program is to provide an extra layer of support and care for people living with chronic conditions who are at risk of rehospitalization.

Typically, skilled nursing centers can send up to 15% of their patients home each month. Patients often experience disruptions in their pharmaceutical care upon returning home, confusion about their new medication regimen, and the physical challenge of traveling to a pharmacy to access their medications. With pharmaceutical home care in place, the patient avoids this disruption, reduces their medication-related adverse events, and ultimately reduces unnecessary hospital readmissions or emergency room visits.

This program helps partner facilities provide a better continuum of care and reduces the burden of medication management on discharge planning teams. Early studies indicate that Centennial’s support reduces readmissions to less than 9%, increases adherence measures for diabetes and cholesterol medications to over 98%, and has 97% patient retention, with high patient, provider and facility satisfaction.
